Very nice place! I use to work there many years ago and I miss it a lot. The staff was excellent and all worked very well together. Providing excellent patient care was our top priority! I've learned a lot and gained the proper skills and experience to carry with me throughout my career in the Healthcare industry. I want to say thank you for all the good memories. ?

Worst place to take your loved ones, after the virus hit the stopped caring about the patients. Its called a rehabilitation center for a reason my grandmother has been there for over a month and they only took her out of the bed one time. ONE TIME in the whole month, how is she supposed to recover if she's laying in bed all day. Staff act like they care but once you're done visiting they go back to just talking for hours in the halls and not caring for patients.

We have been blessed with some of the sweetest nurses and aides taking care of my loved one. The last few months were difficult and the social worker was so kind and helpful making sure we had help at home. The equipment we ordered was delivered the same day and people showed up the very next day. This was beyond our expectation I only wish she could have stayed longer at the nursing home, the insurance companies are so quick to discharge. Thank you to all that helped my mother!

I used to work there as part of the therapy company, but I quit. I was even paid more than other long-time therapists who worked there, but I was not happy knowing what was going on. The therapists will do the bare minimum in treating the patients only to be able to bill for the certain therapy medicare codes. Some therapists do not even visit the patients, but end up billing for it, or lie about how many minutes they are to be seen. The company they are using which is Therapy Management Inc. I think is the most corrupt therapy company in Michigan. ... My patients loved me, I was very empathetic and would do my best to help people. If you bring your family member here, they will more than likely not get better, and either be kept for unnecessary reasons or discharged too early with little progress. The staff there has their own reasons to decide everything themselves and then fool the family into thinking something else. It's just a business there. I'm so happy to be working where I am now. ... If your considering this place, be wary of therapy staff not knowing what they are talking about but making it sound like they do, and also either evading or manipulating situations to make it seem like they are doing what they can, trust me, they're not.

Communication there is none. One department does not talk to another. No one takes responsibility for anything it is always someone else's job. I choose this place because I thought it would be an extension of Oakwood the hospital let me be clear " it is not". My mother was discharged December of 2017 and we are just receiving documents to sign in March of 2018. She was there for 30 days and no one realized that the documents were not signed, that is a serious flag. Then they did not realize that she has dual insurance I had to call back and provide it. When I asked why they did not have the information they blamed it on the hospital. Well the hospital billed both insurance companies so I am sure they were not the problem. The nurses and PT staff were great. Everything on a administrative level needs a complete overhaul. My mother will not be back and it is sad because I trust Oakwood the hospital without any reservation. As a matter of the fact the hospital should be concerned that all there good work goes out the window once a patient transfers from the hospital to the rehabilitation facility. Send your loved ones somewhere else if you are like me you already have enough to worry about.

Compare The Number of Problems
Types of Problems at Nursing Homes
Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.
Residents Affected | |||
Severity of the Deficiency | Few | Some | Many |
Immediate jeopardy to resident health or safety | J | K | L |
Actual harm that is not immediate jeopardy | G | H | I |
No actual harm with potential for more than minimal harm that is not immediate jeopardy | D | E | F |
